1.  How do I apply and what forms do I need for state certification as a new chemical dependency treatment provider?

You can request an application packet for DASA certification as a new chemical dependency treatment provider by any of these methods:

2.  What if I move my agency? How do I obtain certification or approval to provide services at another location when I am already a DASA certified chemical dependency service provider?

Certified providers are required to give the department a minimum of 60 days written notice of intent to relocate certified services. Certification is not transferable from a certified location to another site without departmental approval. (See WAC 388-805-110(2) for information on Relocations.)

You can request an Agency Relocation Approval Request Form and Americans with Disabilities Act Checklist for Existing Facilities by using the contact address, telephone, e-mail, or fax previously identified on the first page.

3.  How do I obtain department approval of the change in ownership if I am already a state certified chemical dependency treatment provider, and I want to sell my business to another party?

Certified providers are required to give the department a minimum of 60 days written notice of intent to change ownership of a certified provider.  (See WAC 388-805-105 or the WAC Implementation Guide for information on ownership changes.)

4.  How long will it take the department to respond to my request(s)?

Response times can vary due to workloads. We are committed to responding to each request within 30 days of receipt. We will do our best to keep you advised of delays, if any. There are occasions that the request is unique and requires special consideration, or required information was not provided with the request, or the information provided did not meet the requirements. Incomplete applications or requests can result in a delay or, in some instances, even a denial of the request.

We try to provide forms that explain what information and materials are necessary to complete your request. We encourage you to contact us if you have any questions or concerns prior to submitting your request.
