Error on initial release of TARGET form DSHS 04-416.  The error was corrected on the web on 7-18-2005.  The error is on the last page when the ASAM placement and one other question were duplicated or appeared twice on that page.  Those who downloaded this form between 1-18 July should revisit the web site and download the corrected copy.  Only the last page was affected.

DOC Field Information

Because of state legislative requirements, effective July 1^st, there are three new fields on the Target Data Elements form (04-416) as well as in the Target system.

Court Ordered

This refers to a client that is under Department of Corrections (DOC) supervision. Examples of this would be clients who are under a work release program, are currently incarcerated or under other DOC community supervision.

If the client indicates that they are under DOC supervision then the treatment agency needs to contact the DOC Hotline at (360) 586-6358 to obtain the information for the following two fields.

This field does not refer to Drug Court clients 

DOC Consent Date

If the client has signed a release of information form with the Department of Corrections then that Consent Date is to be entered on the Target form and in the Target system. 

Statutory Max Date

This is the date that Department of Corrections supervision ends.

DOWNTIME

The TARGET system will not be available the last weekend of each month from 5:00 p.m. Friday, through 8:00a.m. Monday.  The dates for this month are: August 26th thru the 29st.
