TARGET Forms

The following forms are under a second revision to reflect a new program of Gambling treatment.  The change does not affect the base forms but adds an end or exit referral source of Gambling to the assessment, discharge and short Detox forms.  The Agency Support activity form has a new activity type of Case Consultation to accommodate out station staff at the child protection offices statewide.  Since we had to update the Assessment Admission form, we corrected the oversight on page 7 and added the section for signature and date to that page.  We anticipate a 12/1/2005 release date of the forms below but will advise when actually available for download from  http://www1.dshs.wa.gov/msa/forms/eforms.html

Change to M3-Client Demographics in Treatment

This report calculates all clients that are in treatment within the date range specified on the report.  That is, the clients had an open admission within the time frame.  This is slightly different than the C3 Open Admissions report, which displays clients that are open on the date specified in the start date field. 

Downtime

The TARGET system will not be available the last weekend of each month from 5:00 p.m. Friday, through 8:00 a.m. Monday. The dates for this month are: November 18th thru the 21st.
