ACES Generated Forms

The following is a list of forms that can be printed from ACES.
  • Application for Benefits (AFB )

  • Appointment Notice

  • Notice of Missing Verifications

  • Request for Benefits (RFB )

  • SSA / SSN Referral

These forms are either printed in the local office or printed in batch that night and mailed to the client.

Application for Benefits (AFB)

  1. The AFB contains all the data collected for an application or eligibility review that is used to determine initial and ongoing eligibility.

  2. The AFB is printed when the client has not completed and signed a pre-printed application form.

  3. To print the AFB at the local printer from the (DONE), enter [01] in the CLIENT INDICATOR field in the upper right hand corner next to the 4 letter screen name and press <F20>.

  4. The AFB print can only be requested from the (DONE) while in interim change, Option (R) or interview, Option (O).

  5. After printing the AFB, the user reviews it with the client to confirm that the data is accurate and asks the client to sign it.  See EA-Z Manual - Applications - E. - Completing the Application Process -  WAC 388-406-0050

Appointment Notice

  1. Schedule the appointment.  See Scheduling.

  2. When screening a case in ACES through Option (J) - Screening - from the (AMEN), the (SCDI) is the last screen in the screening process.

  3. On the (SCDI), press <F13> to go to the (NOTE).

  4. Enter Notice [0011] - Appointment Notice - in the NOTICE TYPE field.

  5. Press <TAB>.

  6. The cursor moves to the beginning of the field where up to two lines of freeform text can be entered.

  7. Information entered here prints in the ADDITIONAL INFORMATION section of the Appointment Notice.

  8. <TRANSMIT> and the (SCDI) displays again.

  9. Press <F22> to print the appointment notice in the local office or <TRANSMIT> to have the appointment notice printed in batch that night.

  10. Even if the appointment notice is printed in the local office, a second copy will be mailed to the client that night in batch.

Notice of Missing Verification

  1. Outstanding verification is listed on the (VERF) screen.

  2. Notice 0023 - A Notice of Missing Verifications - lists items that must be provided and/or verified before the case can be finalized and the data committed.

  3. The (VERF) is the last screen displayed during the interview process.

  4. The user can enter freeform text on the Notice of Missing Verifications.

  5. The Notice of Missing Verifications can be printed from the (VERF) in two different ways.  One of these methods must be selected in order for the form to be printed:

    1. To have it print in the local office press <F20>,

    2. To have it print and mailed that night in batch press <F19>.

Request for Benefits (RFB)

  1. The RFB contains data that is collected from a client during screening.

  2. The RFB is printed when the client has not completed and signed a pre-printed request for assistance (sometimes referred to as Part 1 of the Application Form - DSHS 14-001(X)).

  3. The RFB is printed at the local printer by pressing the <F20> key on the (INCH). The (INCH) is displayed when adding a person, adding a program, or screening.

  4. If the RFB is not printed during screening, it cannot be printed. When this happens, the client must complete Part 1 of the Application Form - DSHS 14-001(X).

  5. If the RFB is not printed at the local office, it will not print in batch that night.

  6. After printing the RFB, the user reviews it with the client to confirm that the data is accurate and asks the client to sign it.  See EA-Z Manual - Applications - E. - Completing the Application Process for WAC 388-406-0050.

SSA / SSN Referral

  1. The SSA / SSN Referral form is given to the client to either take to SSA to verify their SSN or used to request a SSN if the client does not already have a SSN,

  2. A SSA / SSN referral form can be printed in the local office by entering [P] in the SSA/SSN REFERRAL field on either:

    1. The (MEMB) in Screening, or

    2. The (DEM1) in Interview or Interim / Hist. Change.

  3. After the information is committed in either screening or interview, the system changes the (P) to (S) - Sent Referral Form to Client - in the SSA/SSN REFERRAL field on the (DEM1).
     


Printing Forms

  1. To print a form in the local office you must have a printer defined in ACES.

  2. A default printer can be assigned by having your ACES Security Monitor contact the ACES HELP Desk.

  3. If you want your printed ACES forms to go to a different printer, or if no printer ID displays on the (WMEN), then enter the printer ID in the printer ID field on the (WMEN).

  4. If the head of household's primary language code is English, or one of the non-supported languages, the forms are printed in English.

  5. If the head of household's primary language code is one of the supported languages, it is printed in the primary language as well as English.

  6. Inserted text in forms printed locally will print in English and will need to be translated before giving or mailing the form to the client.
