Purpose: Effective June 1, 2005, clients, 65 years and older who receive SSI or SSI-related Medicaid and Medicare living in King and Pierce Counties may enroll in the Medicare/Medicaid Integration Program (MMP) managed care project with Evercare Premier. Evercare Premier provides medical and long term care services through a primary care provider (PCP) and a care management team. These services are provided by a network of contracted health care and long term care providers. The goal of MMP is to increase coordination, provide greater access to services and improve health outcomes while more efficiently utilizing state funds. Clients must contact an Evercare Premier representative to enroll in the program by calling: 1-866-266-0636. |
