SSI Facilitation - Alleged Medical Provider Misconduct

When alleged misconduct by a medical provider is reported:
  1. Report the alleged inappropriate language or actions to the Social Services Supervisor.

  2. Ask the client to write an account of the incident including:

    1. Name, address and telephone number of medical provider;

    2. Date, time and address of appointment; and

    3. Details of the alleged inappropriate language or action.

  3. Send the written account with the name of a local CSO contact to:

  1. Send a copy of the written account to:
