OLYMPIA -- The state Department of Social and Health Services (DSHS) has issued a stop placement of residents at Miranda's Brother's Home Care, a privately operated adult family home at 314 South 8th Street, Walla, Walla.

The adult family home is licensed and owned by Mariana Miranda.

The state took the action after finding numerous violations of regulations that affected the safety and well-being of residents. The violations include but are not limited to:

• The provider lacked understanding of the rules and regulations about operating an adult family home and lacked the ability to meet management and administrative requirements.
• The provider failed to ensure one caregiver was qualified to perform nurse delegated tasks.
• The provider failed to ensure one employee's criminal background results were returned before having unsupervised access to the residents and that a second employee had completed a disclosure statement.
• The provider failed to ensure three residents' records were kept in the home, kept confidential, protected from loss and unauthorized use, and were available to caregivers and department staff.
• The provider failed to complete assessments before admitting three residents to the home.
• The provider failed to ensure four residents had a negotiated care plan within 30 days of admission into the home.
• The provider failed to ensure two residents received medications and/or treatments from an appropriate professional.
• The provider failed to ensure they had written approval from one resident's physician for a nutritional supplement.
• The provider failed to ensure a system was in place to ensure two residents' medication logs were kept up to date to reflect all of the prescribed and over the counter medications, the dosage, the frequency, and time the resident is to receive the medication.
• The provider failed to ensure a system was in place to ensure one resident's medication log was updated reflecting changes in medications, the date of the change, and the new medication was received from the pharmacy.
• The provider failed to ensure a system was in place to ensure three residents received their medications as required.
• The provider failed to ensure she completed specialty care training admitting a resident with dementia.
• The provider failed to ensure fire drills were conducted at least every two months and failed to ensure all residents participated in a full evacuation of the home in 2007.
• The provider failed to ensure the names of the staff involved in fire drills were listed on the drill log.

The provider has the right to contest the stop placement by requesting an administrative hearing within 28 days of state notification. It remains in effect pending all appeals. While the home is in stop placement, licensee may not admit new residents or re-admit current residents without department permission. Current residents may remain.
