OLYMPIA -- The state Department of Social and Health Services stopped admissions and suspended and revoked the license of Flora Vista Boarding Home in Tacoma.

The privately operated boarding home is located at 1146 North James Street. It was licensed and owned by Gina Chae.

The state took action after finding numerous violations. The violations include but are not limited to the licensee failure to:

• Provide care and services that safely and appropriately served one resident.
• Ensure that three residents received medication assistance from a trained and qualified boarding home staff person.
• Ensure that unsupervised workers obtained criminal background checks.
• Ensure that daily boarding home operations – including hiring and firing decisions and care giving duties – were conducted by qualified staff.

The provider has the right to contest the stop placement of admissions, revocation, and summary suspension by requesting an administrative hearing within 28 days of state notification.

New residents cannot be admitted pending the appeal. Current residents are moving to other settings.
