License revoked and admissions stopped at Lacey adult family home

OLYMPIA -- The state Department of Social and Health Services has stopped new admissions and revoked the license of an adult family home in Lacey.

The privately operated Colonial House Adult Family Home Care is located at 4810 18th Avenue. Sean Lewis owns the home and holds the license.

The action was taken after the state found that regulations were violated that affect the safety and well-being of residents. The violations include but are not limited to the licensee's failure to:

The provider has the right to contest the state's actions by requesting an administrative hearing within 28 days of state notification. Residents may stay in the home pending resolution of the appeal process.

However, new admissions or re-admissions are prohibited without DSHS permission.

DSHS will continue to monitor residents' safety if they choose to remain there.
