OLYMPIA -- The state Department of Social and Health Services has stopped placement and revoked the license of Jefferson Park Adult Home Care in Seattle.

The privately operated adult family home is located at 1902 South Spokane Street and is licensed to Zenaida Christman.

The state took the action after finding numerous violations that affected the safety and well-being of residents. The violations include but are not limited to the licensee's failure to:

• Meet the needs of the vulnerable adults in her care.
• Call a doctor, retake blood pressure readings or monitor four residents whose blood pressure was outside normal limits. This placed the residents at risk of dizziness, damage to vital organs, stroke or death.
• Comply with three of four conditions imposed on the license, placing vulnerable residents at physical risk.

While the home is in stop placement, licensee may not admit new residents or re-admit current residents without department permission pending any appeal.

The provider has the right to contest the stop placement and revocation by requesting an administrative hearing within 28 days of state notification. Current residents may stay in the home pending completion of the appeal process. If a resident chooses to remain, DSHS will continue to monitor the home for resident safety.
