OLYMPIA – The state has stopped admissions and revoked and summarily suspended the license of Coral Lane AFH, a privately operated adult family home at 7402 Coral Lane SW, Lakewood.
The adult family home is licensed to Caroline Kang.
The state Department of Social and Health Services took the action after finding violations that adversely affected the safety and well-being of residents. The violations include but are not limited to:
- Failure to ensure staff had current criminal background checks. This is a repeat or uncorrected deficiency previously cited on Aug. 24, 2000, March 25, 2003, Sept. 24, 2004, March 15, 2007, and Oct. 29, 2008.
- Failure to develop and complete a negotiated care plan for two newly admitted residents within 30 days of admission, leaving them at risk of lacking medical care and services, personal preferences and individual choice. This is a repeat or uncorrected deficiency previously cited on Oct. 29, 2008.
- Failure to ensure that two newly admitted residents had care plans that determined what amount of medication assistance they required. They also did not have medication logs to show they had received any medications. This is a repeat or uncorrected deficiency previously cited on Dec. 17, 1998, Aug. 24, 2000, Sept. 29, 2003, and Oct. 29, 2008.
- Failure to review resident assessments to determine the need for side rails or whether the home had informed consent for two residents with side rails. This is a repeat or uncorrected deficiency previously cited on Oct. 29, 2008.
- Failure to ensure two residents' bedrooms inspected, licensed and approved for use. This is a repeat or uncorrected deficiency previously cited on Oct. 29, 2008.
- Failure to ensure the internal environment was free of toxic hazards. This is a repeat or uncorrected deficiency previously cited on Dec. 17, 1998, Aug. 24, 2000, and Oct. 29, 2008.
- Failure to ensure the home's temperature was kept at least 68 degrees during waking hours. This is a repeat or uncorrected deficiency previously cited on Oct. 29, 2008.
- Failure to ensure the home had an adequate emergency food supply. This is a repeat or uncorrected deficiency previously cited on Oct. 29, 2008.
- Failure to ensure the home had adequate emergency medical supplies. This is a repeat or uncorrected deficiency previously cited on Oct. 29, 2008.
While the home is in stop placement, licensee may not admit residents.
The provider has the right to contest the stop placement, revocation and summary suspension of license by requesting an administrative hearing within 28 days of state notification. Residents have been moved to other settings.
