OLYMPIA -- The state Department of Social and Health Services has stopped admissions to Golden Home Care adult family home in Renton.

The privately operated adult family home is located at 3514 Morris Avenue South. The home is licensed to Editha Valdez.

The state took the action after finding numerous violations that jeopardized the safety and well-being of residents. The violations include but are not limited to:

• Failure to have enough staff in the home to meet residents' care needs.
• Failure to ensure a resident had timely personal hygiene care for incontinence. The resident was at risk of skin breakdown.
• Failure to administer pain medication as directed by the medical practitioner.
• Failure to maintain food safety and a sanitary environment, placing four residents at risk for accidental poisoning or illness.
• Failure to ensure residents could be safety evacuated from the home in five minutes or less, and placed them all at risk for harm by not being prepared during an emergency.

While the home is in stop placement, licensee may not admit residents.

The provider has the right to contest the stop placement by requesting an administrative hearing within 28 days of state notification. Current residents may stay in the home. If a resident chooses to remain, DSHS will continue to monitor the home for resident safety.
