OLYMPIA -- The state Department of Social and Health Services has stopped admissions to James adult family home at 2028 SE 251st Place, Kent.

The privately operated adult family home is licensed to Stevan Bachici.

The state took the action after finding numerous violations that jeopardized the safety and well-being of residents. The violations include but are not limited to:

• Failure to manage finances for the home.
• Failure to adequately staff the home, causing a resident to be locked outside in the cold for two hours.
• Failure to ensure the care and services for three residents were provided in way that supported the safety of each resident and to ensure that residents were safe to smoke independently.
• Failure to ensure five staff members had necessary qualifications and valid credentials.
• Failure to ensure three residents received medication assistance as assessed.
• Failure to ensure emergency evacuation drills occurred at least every two months.
• Failure to ensure licensing conditions were followed. This allowed one individual to have unsupervised access to two other residents.

While the home is in stop placement, licensee may not admit residents.

The provider has the right to contest the stop placement, revocation and summary suspension by requesting an administrative hearing within 28 days of state notification. Residents have been moved to other settings.
