OLYMPIA -- The state Department of Social and Health Services has halted admissions to Laurel Park Manor, a privately operated adult family home located at 5010 NE 52nd Street, Seattle.

The home is licensed to Michael Dorsey.

The state took the action after finding numerous violations that jeopardized the safety and well-being of residents. The violations include but are not limited to the provider's failure to:

• Review and update a resident assessment when the negotiated care plan no longer met the current care needs. This is a repeat or uncorrected deficiency previously cited on Jan. 14, 2009.
• Ensure a resident had a nurse delegate to administer Lorazepam (Ativan) and Nitroglycerin, which were prescribed as needed. This is a repeat or uncorrected deficiency previously cited on Jan. 14, 2009.
• Ensure that a medication log was initialed by the person who administered the medications. Because of that, the resident could have missed medication doses prescribed by the doctor.
• Ensure caregivers were within hearing distance from residents so they could hear when residents needed assistance or when a one resident left the home. The resident who left sustained a head injury after falling outside on a cold night. This is a repeat or uncorrected deficiency previously cited on Jan. 14, 2009.

While the home is in stop placement, licensee may not admit residents.

The provider has the right to contest the stop placement by requesting an administrative hearing within 28 days of state notification. Current residents may stay in the home. If a resident chooses to remain, DSHS will continue to monitor the home for resident safety.
