OLYMPIA -- The state Department of Social and Health Services has stopped admissions to Oasis, a privately operated adult family home, located at 5910 West Red Could Court, Spokane.
The home is licensed to Lauretta Valenciano.
The state took the action after finding numerous violations that jeopardized the safety and well-being of residents. The violations include but are not limited to the provider's failure to:
- Ensure three residents had negotiated care plans in place within 30 days
- Involve a physician when she changed the time for administering a resident’s medications. The provider also failed to involve a nurse when performing blood sugar checks, causing a resident to receive mediation at the wrong time. This is a repeat or uncorrected deficiency previously cited on Feb. 15, 2007, March 31, 2008, and June 12, 2008.
- Ensure medication records were kept up to date for three residents. This is a repeat or uncorrected deficiency previously cited on June 12, 2008.
- Ensure the home was maintained in a clean and homelike environment for five residents.
- Ensure two residents received information about services in the home, charges for services, house rules, caregiver qualifications and resident rights.
- Ensure two residents had negotiated care plans for the use of a Hoyer lift and side rail and information related to safety risks caused by the devices. This is a repeat or uncorrected deficiency previously cited on Feb. 26, 2008.
- Ensure the home was maintained and free of hazards for five residents. his is a repeat or uncorrected deficiency previously cited on June 12, 2008 and July 31, 2006.
- Ensure first aid supplies were kept in the home.
While the home is in stop placement, licensee may not admit residents.
The provider has the right to contest the stop placement by requesting an administrative hearing within 28 days of state notification. Current residents may stay in the home. If a resident chooses to remain, DSHS will continue to monitor the home for resident safety.
