OLYMPIA -- The state Department of Social and Health Services has stopped admissions to Billaflores LLC, a privately operated adult family home located at 12058 69th Avenue South, Seattle.

The home is licensed to Alfredo Flores Jr.

The state took the action after finding numerous violations that jeopardized the safety and well-being of residents. The violations include but are not limited to the licensee's failure to:

• Ensure sufficient, qualified staff was on duty.
• Ensure residents had the care and services necessary for optimal physical health, ensure residents were safe and ensure that three residents received care from a medical professional. One resident was harmed when he fell out of his wheelchair, and inadequate attention to his hygiene needs caused a skin condition to worsen. Two other residents were at risk for harm related to undetected and unreported changes in their vital signs and one resident was additionally at risk for harm if blood sugar levels were not monitored. This is a repeat or uncorrected deficiency previously cited on Oct. 5, 2007.
• Ensure all people living in the home could be evacuated in five minutes during an emergency.

While the home is in stop placement, licensee may not admit residents.

The provider has the right to contest the stop placement by requesting an administrative hearing within 28 days of state notification. Current residents may stay in the home. If a resident chooses to remain, DSHS will continue to monitor the home for resident safety.
