OLYMPIA -- The state Department of Social and Health Services stopped admissions and revoked the license of La Casa Marchese, a privately operated adult family home, located at 5405 N Belt, Spokane.
The adult family home is licensed to Joseph Marchese.
The state took the action after finding numerous violations that jeopardized the safety and well-being of residents. The violations include but are not limited to the provider's failure to:
- Ensure that three caregivers had enough training to clearly understand the needs of three residents. This resulted in one resident receiving blood sugar checks from untrained and unqualified caregivers, another resident to receive too much narcotic pain medication and contributed to increased skin breakdown for one other resident. This is a repeat or uncorrected deficiency previously cited on May 13, 2008.
- Obtain background checks for two new employees. This is a repeat or uncorrected deficiency previously cited on May 13, 2008.
- Report potential exploitation of a resident after discovering the resident's narcotic medication supply was missing and report that a resident was missing from the home.
- Immediately notify a resident's family members when the resident's health condition significantly changed.
- Adequately prevent skin breakdown and notify the resident's doctor of the condition and ensure medically qualified personnel were available to provide assistance. Because of this, one resident experience increased skin breakdown, pain and the risk of infection. Another didn't get blood-sugar checks and another received medications from an unqualified caregiver. The provider also failed to ensure that one resident received pain management as ordered. This is a repeat or uncorrected deficiency previously cited on June 30, 2008, May 13, 2008, and July 10, 2007.
- Ensure three residents had their medication properly tracked. This placed the residents at risk for sleep problems, pain and under or over dosages of medication.
- Ensure that the medication logs were kept up to date for four residents.
- Ensure that physician directions for medications prescribed for two residents were followed.
- Ensure that a qualified person filled medication organizers for four residents. This is a repeat or uncorrected deficiency previously cited on May 13, 2008.
- Use dignity and respect while caring for a resident.
- Ensure bedroom doors used by four residents as well as the front door of the home, opened without any special knowledge or effort, placing residents in risk of entrapment in the event of a fire.
- Limit electronic monitoring to the residents' bedrooms and obtain consent for using the device.
- Be truthful about caregiver training, by falsifying training documentation, and be to truthful about obtaining background checks.
The provider has the right to contest the stop placement and revocation by requesting an administrative hearing within 28 days of state notification. Current residents may stay in the home. If a resident chooses to remain, DSHS will continue to monitor the home for resident safety.
