OLYMPIA -- The state Department of Social and Health Services has stopped admissions and revoked the license at Our House, a privately operated adult family home located at 7016 89th Place NE, Marysville.

The adult family home is licensed to Dyna Packard.

The state took the action after finding numerous violations that jeopardized the safety and well-being of residents. The violations include but are not limited to failure to:

• Ensure that staff had the required minimum caregiver qualifications
• Ensure that staff had successfully completed criminal history background checks before leaving them unsupervised with two vulnerable adults. This is a repeat or uncorrected deficiency previously cited on October 24, 2008, November 9, 2007, and August 29, 2007.
• Ensure that caregivers and authorized DSHS staff had a working phone number or access to the provider during an absence from the home.
• Report to the department when a resident was missing from the facility. This is a repeat or uncorrected deficiency previously cited on October 24, 2008.
• Cooperate with department staff during an investigation. The provider and the staff failed to provide consistent, verifiable information.

While the home is in stop placement, licensee may not admit residents.

The provider has the right to contest the stop placement and revocation by requesting an administrative hearing within 28 days of state notification. Current residents may stay in the home. If a resident chooses to remain, DSHS will continue to monitor the home for resident safety.
