OLYMPIA -- The state Department of Social and Health Services has stopped admissions to Lakeside AFH, a privately operated adult family home located at 16011 Eastshore Drive, Lynnwood.

The home is licensed to Gretchen Dhaliwal.

The state took the action after finding numerous violations that jeopardized the safety and well-being of residents. The violations include but are not limited to the failure to:

• Immediately notify a resident's physician or case manager following a significant change in the resident's physical condition.
• Immediately contact emergency medical services for a resident who had two medical emergencies and ensuring medical personnel were immediately provided a copy of the resident's medical care directive.
• Ensure the home had a safe medication system. The home failed to destroy medications no longer used, failed to ensure two residents received medications as prescribed and failed to ensure the medication log was current with all the required information for two residents. There were documentation errors, medication errors and unaccounted narcotic medications for former and current residents.
• Ensure medications were properly set up before four residents took their medications and did not ensure all medications were in locked storage in the original labeled container.

While the home is in stop placement, licensee may not admit residents.

The provider has the right to contest the stop placement by requesting an administrative hearing within 28 days of state notification. Current residents may stay in the home. If a resident chooses to remain, DSHS will continue to monitor the home for resident safety.
