OLYMPIA -- The state Department of Social and Health Services has stopped admissions to and revoked license of Allen Homecare, a privately operated adult family home, located at 12742 NE 132nd Place, Kirkland.

The adult family home is licensed to Julia Allen.

The state took the action after finding numerous violations that jeopardized the safety and well-being of residents. The violations include but are not limited to failure to:

• Make necessary notifications for two residents who had significant medical events.
• Provide care services for three residents. One resident fell and sustained multiple fractures, another resident fell and caught her arm in a bed rail and another was improperly transferred by care staff. This is a repeat or uncorrected deficiency previously cited on April 22, 2008.
• Complete a needs assessment and ensure staff was trained before using bed rails.
• Additionally the licensee knowingly made a false statement and willfully interfered with a DSHS investigation, when she stated and documented she had notified a physician when a resident fell and sustained multiple injuries.

While the home is in stop placement, licensee may not admit residents.

The provider has the right to contest the stop placement and revocation by requesting an administrative hearing within 28 days of state notification. Current residents may stay in the home. If a resident chooses to remain, DSHS will continue to monitor the home for resident safety.
