OLYMPIA – The state Department of Social and Health Services has stopped admissions and revoked and suspended the license of My Grandma’s House LLC, a privately operated adult family home, located at 1227 South Geiger, Tacoma.

The adult family home is licensed to Cynthia Blancaflor.

The state took the action after finding numerous violations jeopardized the safety and well-being of residents. The violations include but are not limited to:

• The home failed to meet the financial obligations required to own and operate a business resulting in an abrupt closure of the home with five current residents. The residents were at immediate risk of homelessness and the lack of shelter, food, care and services. This is a repeat or uncorrected deficiency previously cited under WAC 388-76-10960 (18) from the Statement of Deficiencies dated May 7, 2008.

The provider has the right to contest the stop placement, revocation and summary suspension by requesting an administrative hearing within 28 days of state notification. Current residents have been moved to other settings.
