OLYMPIA – The state Department of Social and Health Services has stopped admissions and revoked the license of Lynnwood Care AFH Inc., a privately operated adult family home, located at 5707 173rd Place SW.

The home is licensed to Beniamin Ungureanu. The license number is 750659.

The state took the action after finding numerous violations that jeopardized the safety and well-being of residents. The violations include but are not limited to:

• Not immediately notifying a doctor or case manager when the condition of two residents significantly changed.
• Making an error in dispensing medication because no one had delegation authority for administering medication and a medicated cream. This is a repeat or uncorrected deficiency.
• Not having a system in place to ensure the medication services met the assessed needs of residents; did not ensure a current medication list was in the record; did not keep an up-to-date medication log with the required information; did not ensure medication orders were verified; did not ensure the new medication was received from the pharmacy and/or did not ensure medications were received as required for three residents. This failure resulted in medication error for two residents and may have resulted in medication error for one resident.
• Not complying with conditions on the license, placing all resident at risk.

While the home is in stop placement, licensee may not admit residents.

The provider has the right to contest the stop placement and revocation by requesting an administrative hearing within 28 days of state notification. Current residents may stay in the home. If a resident chooses to remain, DSHS will continue to monitor the home for resident safety.
