OLYMPIA – The state Department of Social and Health Services has stopped admissions to Sarausad House, a privately operated adult family home, located at 2108 NW 98th Street.
The home is licensed to Bernardita Sarausad. The license number is 484500.
The state took the action after finding numerous violations that jeopardized the safety and well-being of residents. The violations include but are not limited to:
- Failure to provide adequate dementia care. This is a repeat or uncorrected deficiency.
- Failure to ensure necessary care and services were provided for two residents. One resident had an unplanned weight loss of 12 pounds in two months. The other resident became unable to bear full weight on her right foot and did not have the benefit of a medical examination. This is a repeat or uncorrected deficiency.
- Failure to ensure three residents received meals at regular times comparable to normal mealtimes in the community. The residents received only a small snack at 6:30 p.m. This is a repeat or uncorrected deficiency.
- Failure to ensure a resident was aware of changes in her medication. The provider lacked documentation showing there was consultation with a doctor or pharmacist regarding the medication change and lacked documentation showing the resident was aware of the change. This is a repeat or uncorrected deficiency.
- Failure to ensure that a dementia care specialist worked with the provider and staff to develop and train the licensee and staff on appropriate behavior interventions.
While the home is in stop placement, licensee may not admit residents.
The provider has the right to contest the stop placement by requesting an administrative hearing within 28 days of state notification. Current residents may stay in the home. If a resident chooses to remain, DSHS will continue to monitor the home for resident safety.
