OLYMPIA – The state Department of Social and Health Services has stopped admissions and revoked the license of Salmon Creek Adult Care, a privately operated adult family home, located at 12903 NE 3rd Avenue, Vancouver.
The adult family home is licensed to Kimberly Coonrod Siddiqui. The license number is 521900
The state took the action after finding numerous violations that jeopardized the safety and well-being of residents. The violations include but are not limited to:
- The licensee failed to understand that she was responsible for meeting the physical and special needs of five residents living at the adult family home and be in compliance with the minimum licensing requirements. This made residents vulnerable to mistreatment, further health complication and diminished quality of life.
- The provider failed to ensure a resident's medical needs were met by not obtaining an assessment before the resident moved into the home.
- The licensee failed to ensure a significant change of condition assessment was completed for one resident, leaving him vulnerable to not having his ongoing needs identified and met.
- The licensee failed to ensure resident services were delivered, causing a resident to receive medication not as ordered by a medical professional.
- The licensee failed to ensure five residents received nurse delegation services as needed, allowing residents to receive services from untrained and unsupervised caregivers. Services for residents were delayed.
- The licensee failed to notify residents and their representatives that the home no longer had either a nurse on staff or around-the-clock access to an on-call nurse.
While the home is in stop placement, licensee may not admit residents.
The provider has the right to contest the stop placement and revocation by requesting an administrative hearing within 28 days of state notification. Current residents may stay in the home. If a resident chooses to remain, DSHS will continue to monitor the home for resident safety.
