OLYMPIA -- The state Department of Social and Health Services has halted admissions and revoked the license of Rose Manor Inc., a privately operated adult family home, located at 832 Pine Street, Edmonds.

The adult family home is licensed to Sharon Weaver. The license number is 547600.

The state took the action after finding numerous violations jeopardized the safety and well-being of residents. The violations include but are not limited to:

• The licensee did not provide residents with routine or emergent care when she went off duty from 9:30 p.m. to 9:30 a.m. The made residents vulnerable to harm by not meeting their physical and psycho-social needs.
• The licensee failed to administer physician-ordered routine narcotic medication to a resident, causing emotional and physical distress. The licensee also failed to return the resident's medication to the pharmacy in a timely manner after the resident left the adult family home. The resident went for an additional six days without the benefit of pain medication.
• The licensee failed to ensure residents' rights were respected when she told all residents that she would be off duty from 9:30 p.m. until 9:30 a.m. the next day. The residents were told they had to be in their rooms and could not watch TV or videos during these hours. This is a repeat or uncorrected deficiency.
• The licensee failed to ensure she met the financial obligations in owning and operating her adult family home business when she went into foreclosure.

While the home is in stop placement, licensee may not admit residents.

The provider has the right to contest the stop placement and revocation by requesting an administrative hearing within 28 days of state notification. Current residents may stay in the home. If a resident chooses to remain, DSHS will continue to monitor the home for resident safety.
