OLYMPIA -- The state Department of Social and Health Services has stopped admissions to Garden Courte Alzheimer Community, a boarding home located at 626 Lilly Road NE, Olympia.
The boarding home is licensed to Olympia Special Care LLC. The license number is 1111.
The state took the action after finding numerous violations that jeopardized the safety and well-being of residents. The violations include but are not limited to:
- The facility failed to assess for safety and keep six residents free of physical restraints and one resident free of chemical restraints. The failure placed residents at risk for a decline in ability to walk, injury, social isolation, falls and decreased quality of life. This was a repeat or uncorrected deficiency.
- The facility failed to ensure negotiated service agreements were updated and reflected the current care and services for five residents. The failure prevented staff from having the information to meet residents' physical and behavioral needs.
- The facility failed to ensure medications were administered and blood sugar checks were performed according to professional standards of practice for 24 residents whose medication administration records were reviewed. This failure placed residents at risk for improper doses of medication, medication side effects and adverse health outcomes.
- The facility failed to ensure six residents had the opportunity to engage in independent or self-directed activities that were consistent with their assessed interests, functional abilities, preferences and incorporated into the negotiated service agreement. Residents were vulnerable to losing mental functioning, a decreased quality of life and boredom.
- The facility failed to ensure enough staff was available to meet the residents' needs as outlined in the negotiated service agreements. Residents were woken up, sat in front of the television for extended periods, sat in urine-soaked clothes and restrained.
While the home is in stop placement, licensee may not admit residents.
The provider has the right to contest the stop placement by requesting an administrative hearing within 28 days of state notification. Current residents may stay in the home. If a resident chooses to remain, DSHS will continue to monitor the home for resident safety.
