OLYMPIA – The state Department of Social and Health Services has stopped admissions to and revoked and summarily suspended the license of Golden Years, a privately operated adult family home, located at 1333 172nd Street NE, Marysville.
The adult family home is licensed to Carey Bauman. The license number is 705100.
The state took the action after finding numerous violations that jeopardized the safety and well-being of residents. The violations include but are not limited to:
- The provider neglected three residents by not ensuring they received necessary care and services including medications. One resident was harmed.
- The provider failed to maintain a clean, sanitary homelike environment for the residents, diminishing their quality of life.
- The provider failed to obtain nurse delegation for one resident who was unable to self-administer insulin or perform glucose monitoring. Untrained caregivers administered insulin and monitored blood glucose, placing the resident at risk of medication errors and unmet health care needs.
- The provider failed to have a system in place to keep medications labeled, separated and locked securely. The provider failed to have a system in place to properly dispose of expired or discontinued medications and medications belonging to residents who no longer live in the home. These failures placed all residents at risk of medication error and unmet care health needs.
- The provider failed to ensure medication administration records were current. This failure resulted in medication omissions, errors and unmet care needs for three residents.
The provider has the right to contest the stop placement, revocation and summary suspension by requesting an administrative hearing within 28 days of state notification. Current residents have been moved to other settings.
