OLYMPIA -- The state Department of Social and Health Services has stopped admissions and revoked the license of Salmon Creek Adult Care LLC, a privately operated adult family home, located at 12907 NE 3rd Avenue, Vancouver.
The adult family home is licensed to Tatiana Kittrell. The license number is 750679.
The state took the action after finding numerous violations that jeopardized the safety and well-being of residents. The violations include but are not limited to:
- The licensee failed to understand her responsibility to meet the physical and special needs of the home's six residents living at the adult family home and to be in compliance with the minimum licensing requirements. The residents' quality of life was diminished and they were at risk for health complications.
- The provider failed to ensure two residents' negotiated care plans were developed within 30 days of admission. This placed the residents at risk of care needs not being met and did not give the resident and/or the representative an opportunity to have input into the care they received.
- The provider failed to ensure one resident received medications as ordered and three residents received nurse delegation services. Medication services were provided by untrained staff, jeopardizing a resident's medical condition.
- The provider failed to serve meals to accommodate a resident's digestive condition and another resident's modified sodium restriction diet. The failure made residents vulnerable to serious medical complications. This is a repeat or uncorrected deficiency.
- The provider failed to ensure medical records were updated and to document medications provided to residents
- The provider failed to ensure information about the availability of nursing services was provided to residents or their representatives. Residents and family members were unclear about who was responsible for operating the home and were misinformed about services available at the home.
The provider has the right to contest the stop placement and revocation by requesting an administrative hearing within 28 days of state notification. Current residents may stay in the home. If a resident chooses to remain, DSHS will continue to monitor the home for resident safety.
