OLYMPIA -- The state Department of Social and Health Services has stopped admission to Evergreen Lodge, a boarding home located at 31002 14th Avenue South, Federal Way.

The boarding home is licensed to Emeritus Corporation. The license number is 1699.

The state took the action after finding numerous violations that jeopardized the safety and well-being of residents. The violations include but are not limited to:

The boarding home failed to implement a safe medication system to support four residents. The facility failed to ensure residents received medication as prescribed, failed to evaluate resident's response to medication, failed to ensure staff accurately documented medications as given, and to ensure medications were available. These failures contributed to an exacerbation of one resident's skin condition and placed all residents at risk of more serious of health conditions. 

The boarding home failed to investigate an incident of suspected verbal abuse. The facility failed to protect residents and prevent future similar incidents of aggressiveness against multiple residents. The verbal abuse and aggressiveness decreased the residents' quality of life.

While the home is in stop placement, licensee may not admit residents. 

The provider has the right to contest the stop placement by requesting an administrative hearing within 28 days of state notification. Current residents may stay in the home. If a resident chooses to remain, DSHS will continue to monitor the home for resident safety.
