Skilled nursing facilities provide 24-hour care for people who need help with daily activities and medical support. They serve older adults, people with disabilities, and those recovering from illness or injury. DSHS partners with facilities across Washington to ensure respectful, high-quality care.

Roads to Community Living
One of Washington’s programs that helps people move from nursing homes and other care facilities back into their homes and communities.
This person-centered program supports individuals with complex, long-term care needs. It promotes independence, dignity, and inclusion—and is part of the federally funded Money Follows the Person initiative.
Eligible participants may include people living in nursing homes, hospitals, or residential habilitation centers who are ready to transition to a qualified community setting.
