This section is for new and existing community instructors who want to get contracted to offer training. Community instructors are businesses or individuals wanting to teach long term care worker training in care settings throughout the community and/or home care agencies wanting to teach their own staff.

AFHs or ALFs ONLY wanting to teach their staff apply differently. Go to the AFH/ALF training application page.

Important Announcement: Continuing Education Curriculum   

  • No more than 24 class hours may be submitted per CE approval form (forms with more than 24 class hours will be returned).
  • Only one CE Application will be accepted at a time (additional CE forms will be returned).

Applying for the FIRST Time to Train

Applying for the first time to offer training? Review all the following steps.

How this is done Form Needed
Fill out and submit a Training Program Community Instructor Application (TPC) form.  This form tells DSHS what courses, instructors, and curriculum you want to use TPC Form 
Step 3. Get curriculum (training materials) approved
How this is done

Choose curriculum (training materials) options.  Curriculum:

Make sure to list the curriculum name in your TPC Form.

Core basic training 
Revised Fundamentals of Caregiving (RFOC) with enhancements
CB-RFOC Form RFOC can be used as the foundation for core basic training but it must be enhanced.  These enhancements must be approved and sent in with the CBRFOC

DSHS has a 3 hour curriculum on challenging behaviors that can be used in core basic training, population specific training, or CE.  List it in your application where you want to use it. Download the Navigating Through Challenging Behaviors curriculum (PDF). Step 4: Get instructors approved

How this is done

Form Needed

  1. Review the DSHS minimum instructor qualifications. Ensure that your instructor(s) meet the minimum qualifications for each course you intend them to teach.  
  2. If adding an instructor for Orientation, Safety training, of continuing education complete the TPC form. Use this form to attest that new instructors meet the minimum qualifications.
  3. If adding an instructor for courses listed below complete the TPC form and complete an INS form for each instructor.
  • Core Basic
  • Population Specific
  • Mental Health Specialty
  • Dementia Specialty
  • Nurse Delegation Core
  • Nurse Delegation Diabetes

If adding an instructor for Mental Health Specialty or Dementia Specialty submit a copy of the instructor managers level training for those courses.

Step 5: Contract with DSHS to offer training
How this is done Form Needed Helpful Information
Fill out and submit the contract intake form.  Contract Intake Form You do not have to send in a copy of a W-9 or a Certificate of Insurance for this particular contract with DSHS.

Update Your Approved Training Program

Already approved to offer classes but need to make changes? Review all the following steps to UPDATE your training program. Click on each link for instructions.

Step 1: Notify DSHS of changes

Fill out and send in a Training Program Application Update form. This form tells DSHS what courses, instructors, and curriculum you want to add or remove. TPC UPDATE Form   Only list changes and updates.
Step 2: Add or remove Instructors

To remove an instructor use the TPC Update form.

To add an instructor:

  1. Review the DSHS minimum instructor qualifications.
  2. If adding an instructor for Orientation, Safety training, of continuing education complete the TPC Update form to attest that new instructors meet the minimum qualifications. Also use this form to list any instructors you want to remove.
