How Do I Apply for a Medicaid Contract

What Is Medicaid

Medicaid is a government health insurance program available to people with very limited income and financial resources. Medicaid sends payments directly to health care providers. Medicaid can pay for medical services to eligible residents who qualify for the program.

Once the Application for an Assisted Living Facility Medicaid Contract is approved by Residential Care Services, a Contract request memo, identifying the Contract type, will be forwarded to the Contracts unit.

After the request is received, the Assisted Living Facility (ALF) Contract manager will send a request to the Contractor to complete and return the following forms:

Note: Please do not return these forms until they are requested by the ALF Contract Manager.

The information provided on these forms will be used to prepare the Medicaid Contract and a validation will be completed for all staff with management, ownership or controlling interest listed to ensure that they meet federal and state requirements as a condition for participation in Medicaid reimbursement.  After the validation is completed, a facility Contract will be prepared and forwarded to the Contractor for signature. Once signed by both parties, the fully executed Contract is returned to the Contractor and they are authorized to admit Medicaid clients into the facility.

  • Email adshqcontracts@dshs.wa.gov with the request for Medicaid Contract with the following information:
    • Facility Name
    • Facility License number
    • Facility address
  • Once the Contracts Unit is notified of the request the following forms will be emailed to the provider for completion; or the provider can complete these forms prior to request and submit completed forms with request for Medicaid contract:
  • Once all forms are returned and processed the contract manager will email provider copy of contract for review and signature. 
  • Provider will return signed copy of contract.
  • Contract staff will sign and return a fully executed copy of contract to provider with notification of ProviderOne number.
  • Healthcare Authority will contact provider with login instruction by email.  If email from HCA is not received within 24hr after fully signed contract is received the provider can contact HCA for login credentials (800-562-3022).

If you would like to become Contracted to provide care to Medicaid clients, you will need to inform your licensor, during the licensing process. When the facility license is issued and Medicaid certification is approved, the licensing unit will forward a Contract request to the Nursing Facility Services (NFS) Contract Manager.

At that time, the Contract Manager will send a request to the Contractor to prepare and return the following required forms:

Once the forms are completed and returned, the Contract Manager will enter the information into the Contracts Database and a Federal validation will be completed for all staff with management, ownership or controlling interest.  After the validation is completed, a facility Contract will be prepared and forwarded to the Contractor for signature. The Contract is then signed by both parties and the facility is approved to accept Medicaid clients.
