The formulation of the State Hospital Discharge & Diversion (SHDD) Head Quarters (HQ) and regional teams were established in response to delays in state hospital transitions. ALTSA received additional funding to address the state hospital transition and diversion needs for individuals who have been determined financially and functionally eligible for Long-Term Service Supports through increased staffing and service supports.
Each region has an assigned SHDD Behavior Support Trainer to offer a variety of supports for providers supporting individuals who have transitioned from State Hospitals.
