Child Abuse and Neglect (CA/N) Founded Findings Requests from External Requestors

The information provided through this service is limited to the existence of founded findings of child abuse and neglect.  “Inconclusive” or “unfounded” findings, or other information contained in the individual’s record, will not be provided through this process. Specific records requests must be made through Children’s Administration Public Disclosure at 1-844-506-8375 or CAPublicDisclosure@dshs.wa.gov upon completion of the CA/N search.

CA/N Founded Findings Request from Another State

  • For a Public Child Welfare agency requesting CA/N history as part of a CPS or Child Welfare investigation, the request must be submitted on their state agency's letterhead and include language indicating the subjects are part of an ongoing investigation.  Identifying information should include:
    • Parents’ current last name, first name, middle initial and maiden name (if any).
    • Parents’ dates-of-birth.
    • Children’s last name, first name & middle initial.
    • Children’s dates-of-birth.
    • SSN’s for all if available.

  • For all other requestors outside of Washington State DSHS #23-041 form must be used. State of Washington DSHS Children's Administration requires a fee of $20 for each inquiry of an individual's child abuse and neglect findings request.
    • Form must be typewritten, dated and signed. Any handwritten or incomplete forms will be returned for correction.
    • Complete one form for each individual for whom a child abuse/neglect findings request is being requested.
    • Include a check in the amount of $20 per individual inquiry, made payable to DSHS Children's ADMN and mail completed requests to:

    • This fee is not applicable to ICPC requests for a Washington dependent child.  Requesting ICPC state  must provide the WA Children’s Administration case number and this completed form sent to CANhistorychecks@dshs.wa.gov or call 1-800-562-5624.

CA/N Founded Findings Requests within Washington State

  • DSHS #23-032 form is to be used only for requestors within Washington State for the purpose of foster home licensing and adoption background check requests.
    • Form must be typewritten and signed. Any handwritten or incomplete forms will be returned for correction.
    • Complete one form for each individual for whom a child abuse/neglect findings request is being requested.

  • For the purpose of employment, volunteering or interning in settings requiring child abuse and neglect history checks, the requesting agency/employer/volunteer organization must provide a release signed on their letterhead stationery. The form must be typewritten. The applicant must sign and date the form that includes the following language: “A CPS search is needed for employment/volunteer/internship purposes.” and “I am waiving my right to confidentiality of information about child abuse and neglect (CA/N) history, including any founded findings of abuse or neglect, and authorizing its release to: (name of agency/employer being released to).

    Include:

    - applicants’ full name, including maiden name if married
    - date of birth
    - social security number
    - current place of residence in the state of Washington.
