WSIRB Forms 

Researchers planning to submit a proposal to the WSIRB should call and discuss their proposed research with Review Section staff before preparing their application. You may call us at 360.902.8075 or contact us via e-mail

The WSIRB implemented charges for its review services beginning December 1, 2015. Review will not be conducted unless charges are paid in full in advance. Additional information and a fee schedule are available at WSIRB Fees and WSIRB Review Fee FAQs links on the left.

All submissions to the WSIRB are received via email.  We confirm all received submissions. Submission of outdated forms are not accepted.

  • Please note the following changes in review procedure:

    The Revised Common Rule becomes effective January 19, 2018; until this date, we will be applying a dual regulatory review policy. All submissions during this period of time may be subject to part or all of the Revised Common Rule. Submissions received prior to January 19, 2018, may be reviewed under either the old Common Rule or Revised Common Rule per the discretion of the reviewing team. Submissions received on or after January 19, 2018, will be subject to the Revised Common Rule. If you have any questions about the transition process, please contact the WSIRB by e-mail (wsirb@dshs.wa.gov) or by telephone (360-902-8075).

  • The WSIRB only reviews research within its jurisdiction: the HRRS and WSIRB only review research that falls within their authority or scope of review, including Washington State agencies: DEL, DOH, DSHS, HCA, L&I, and OFM and, under formal agreement, with local government agencies, including for example: Clark County Public Health, Tacoma-Pierce County Health Department, and Whatcom County Health.
  • Federal regulations require that Continuation Approval Requests be approved for a period of not more than one year from your study's initial or prior WSIRB approval date.
  • E-mail approaching 30 MB in size should be sent after 5 pm: E-mail that exceeds 30 MB are blocked - senders may not receive notification of delivery failure. Please zip large files, or send multiple e-mails with attachments and confirm receipt with us if you send large files. Please let us know how many files to expect.
