What information must be listed in Box #1 on the AFH or BH Background Authorization form?

You must list the exact facility name as it appears on your license. The facility name must match the registered trade name on your master business license. The facility name on your license is the name that is listed in the Department’s tracking system.

You must also list in Box #1 the site address of the facility. The site address is the address where the facility is physically located. DO NOT list the mailing address. If the information listed on the Background Authorization form does not match the site address in the Department’s tracking system the background form will be rejected.

How can I have my background results returned to me?

If you fax a background form to the Background Check Central Unit (BCCU), your results will automatically be faxed to you at the fax number that is listed in the Department’s tracking system. If no fax number is listed in the Department’s tracking system the results will be mailed to you at the mailing address listed in the Department’s tracking system.

If you mail in your background forms to the Background Check Central Unit (BCCU) your results will be mailed to you at the mailing address that is listed in the Department’s tracking system.

The BCCU will NOT fax or mail to a different fax number or address if you write a note on the background form or cover letter. The BCCU MUST use the information that is listed in the Department’s tracking system.

How can I change my mailing address or fax number?

AFH Providers: To change your mailing address or fax number you must provide your RCS Licensor via fax or mail, a written request for the change. The licensor will change the information in the Department’s tracking system.

BH Providers: To change your mailing address or fax number you must provider your RCS Field Manager, via fax or mail, a written request for the change. The field manager will change the information in the Department’s tracking system.

How do I change the name of my facility?

You will need to register the new trade name with Department of Licensing. Master License Service. Contact the Department of Licensing at (360) 664-1400 or go to the website at http://www.wa.gov/dol/dpd/tradenm.htm for trade name information. Provide proof of that registration and a request to change the facility name to your RCS Licensor if you have an AFH or your RCS Field Manager if you have a BH.

What do I do if I get a rejected background form from the Background Check Central Unit (BCCU)?

The letter from BCCU will tell you what was missing or incorrect on the Background Authorization form. You will need to make the corrections and resubmit the form to the BCCU. If you are unsure of the reason, please call the BCCU for clarification at the number listed on the letter.
