2005 BH "Dear Provider" Letters

ADSA: BH #2005-009
BH LICENSE RENEWAL FEE – DISCONTINUATION OF SECOND BILLING NOTICE

Dear Boarding Home Provider:

The purpose of this letter is to inform you of a change that will be occurring related to the billing notices of your license renewal fee. Currently, the Office of Financial Recovery (OFR) sends you the first and second billing notice and coupon for payment of your license renewal fee.

Effective immediately, OFR will stop sending you a second billing notice and coupon. You will receive only one billing notice and coupon for payment of your license renewal fee. This billing notice will be mailed to you on the 15 th of the month that is 60 days prior to the month your license expires. For example, if your license expires November 30, 2005, your license renewal fee is due October 15, 2005 and your billing notice and coupon will be mailed to you on August 15, 2005. You will not receive a second billing notice and coupon.

The license renewal fee payment and signed coupon continue to be mailed directly to OFR. The Business Analysis and Applications Unit (BAAU) will receive an automated notice that the renewal fee is paid. The BAAU will process the license and mail it directly to the facility.

WAC 388-78A-3230 authorizes assessment of a $10 per day late fee against any licensee who has not paid the license renewal fee at least 30 days before the license expires. The BAAU will notify you in a separate letter if you are assessed late fees.

Remember that changes in licensed bed capacity cannot be made on the new license renewal invoice / coupon. Complete a Change in Bed Capacity form for all changes in licensed bed capacity. All change in capacity fees should be pro-rated using the calculation at the top of the form. The Change in Bed Capacity form is available on the Internet at http://www.aasa.dshs.wa.gov/Professional/bh.htm. Scroll down and click on Change in Bed Capacity or Licensed Rooms form.

Return the completed Change in Bed Capacity form by mail or fax to:
