Health Home Care Coordinator Core Training Materials
In order to become qualified as a Health Home Care Coordinator, eligible staff must complete twelve (12) modules and attend a one-day in-person training.
Attestation to Register for the One-Day – Please use this document to note the credential of staff and the completion date of each module. This form must be completed in order to register for the in-person training session. Once completed, send the form to the lead trainer for the session staff would like to register for. Interested participants should contact their Lead organization for a training schedule.

Additional Training Information
In an effort to provide a quality training experience for all oncoming Care Coordinators and affiliated staff, the following will be adhered to with no exceptions.:
- Class sizes will be limited to 15 individuals or less and will be filled on a first-come/first-service basis.
- Registrants being hired as care coordinators will have first priority for training slots. Affiliated/allied staff can fill remaining slots that are available.
- There will be a 5-business day cutoff period for registering staff for the One-Day Care Coordinator Training. For example, if a training is scheduled for June 10, no registration requests will be accepted after June 5. There will be no exceptions to this protocol.
- Registration slots will not be held for staff in lieu of submitting required training documents (credentials/exception to the rule, Attestation to Attend the One-Day Care Coordinator Training document, etc....). All required paperwork must be submitted at the time of registration.
- The 12 training modules MUST be completed prior to submitting for registration. There are no exceptions to this requirement.
