I have been convicted of and/or have a pending charge; should I disclose it?

Yes, you must disclose all convictions, negative actions, and pending charges.  When signing and submitting the background check form you are swearing under penalty of perjury that it is the whole truth, including all of your answers to all of the self-disclosure questions.  You should disclose all convicted and/or pending charges you have. You can attach a List of Additional Crimes or Pending Charges (DSHS 09-653D)(Word) to your Background Check Authorization Form (DSHS 09-653) (Word) listing the details of the crime, such as the conviction date (mm/dd/year), the crime convicted of, and what state the crime was convicted in. If you have a theft related crime(s), it is also helpful to include the dollar amount.
