REVISIONS TO RAI USER'S MANUAL/MDS TRAINING SESSIONS

Dear Nursing Facility/Home Administrator:

This letter contains important information for facility staff responsible for completing the Minimum Data Set (MDS) assessments.

The Centers for Medicare and Medicaid Services (CMS) recently released the updates to the Long Term Care Facility Resident Assessment Instrument User's Manual Version 3.0. Changes include updates and clarifications, some of which are minor "clean ups" such as correcting spelling, the addition of new items and some substantive changes as well.  Changes have occurred in Chapter 5 and Sections C, G, H, M, P and X of Chapter 3.  Portions of chapters 1, 2, 4, and Appendices have minor edits. The updates are effective May 20, 2013.
The update includes "Track Changes" documents that describe the items in each chapter/section that have changed and the page number of the changes and the actual chapter/sections that have been changed.  You may choose to download the entire chapters or only the pages where changes have occurred.  Your software vendor should be updating your software package to accommodate the changes:

  • If a resident goes from Medicare Advantage to Medicare Part A, the Medicare PPS schedule must start over with a 5 -day PPS assessment as the resident is now beginning a Medicare Part A stay.
  • Item A0800 no longer accepts a dash-filled value to indicate the gender. Item A0800 should match the gender in the Social Security system.
  • Item C0100 adds to "Code 0, no" if the interview should not be attempted because the resident cannot respond verbally or in writing, and includes residents who use American Sign Language (ASL).
  • The algorithm on page G-6 has been updated.
  • Section M has a number of significant changes. Oral Mucosal ulcers caused by pressure should not be coded in Section M. M0300D coding tip "Cartilage serves the same anatomical function as bone. Therefore, pressure ulcers that have exposed cartilage should be classified as a Stage 4."
  • Addition of the word, "palpate" and "increased in numerical stage". MASD definition expanded.
  • Section P has a number of significant changes. The word "physical" has been inserted prior to the word "restraint" throughout this section.
  • A dash must be used in X0300 on a modification or inactivation request to locate a record if a dash was previously entered in A0800 on the original record.
  • Chapter 5 has changes to the modification/inactivation rules.

It is the responsibility of the individuals who complete the MDS to have the most current RAI manual instructions in order to accurately complete the required assessments.
Residential Care Services (RCS) will be providing three remaining 2 day MDS 3.0 Basic seminars. The seminar schedule is listed below and facilities will need to pre-register the number of attendees for each session. Each session will begin at 8 am and end around 4 pm.
