Key responsibilities include
· Provide residents’ training, care and assistance by implementing Individual Instruction Support Plans, Positive Behavior Support Plans, Delegated Nursing Guidelines and Responsibilities, and Individual Financial Plans.
· Provide supervision and monitoring of SOLA participants in accordance with Individuals' Care Plan, IISP, risk protocols and other written plans.
· Assist and train residents in all areas of Activities of Daily Living (ADL's) and personal care, including bathing, dressing, grooming, feeding, toileting, personal hygiene, social behavior and household management on any of the SOLA supported homes.
· Assist residents to seek recreational opportunities that are life enhancing and supports individualized choice. This could be accessing their community, attending church, swimming, dancing, or any variety of activities or events.
· Transport and/or accompany residents to all appointments, and/or activities in the community.
· Assesses residents’ medical and overall conditions. Implement all nursing procedures as trained by clinical/RND nurse. May include, but not limited to medication administration. Blood pressure cuffs/machines, use oximeter, suction and percussion equipment, nebulizer, colostomy care, Internal pump, provide stoma care, program and use a feeding machine and monitor an oxygen compressor.
· Monitor assigned residents at all time. Check on those who are sleeping, at least every 2 hours, and toilet or change, as needed. Turn those who cannot turn themselves, at least every 2 hours. Check skin integrity for signs of breakdown.
· Responsible for maintaining the resident’s home, keeping it safe, and clean, while respecting personal property. Provide lawn maintenance that includes moving, weeding, trimming, watering yard and shoveling snow as needed.
