
Community-Based Care Systems for the Functionally Disabled (CBC) was a research and demonstration project that began in 1975 under United States Department of Health, Education, and Welfare funding and was conducted by the Washington State Department of Social and Health Services (DSHS). Its major purpose was to determine whether an organization geared to meeting the long-term social and health needs of low income aged and functionally disabled adults in a community-based setting would alter usage of and public expenditures for community services and reduce nursing home utilization by Medicaid patients. The concerns that led to this project included (1) the rapidly growing demand for publicly funded nursing home services and (2) the fragmentation of community-based services. The CBC Project was intended to test the prospects of creating an organization to screen and assess functionally disabled persons and to develop a network of community-based social and health services for persons identified as at high risk of nursing home placement.
