State Hospital Discharge and Diversion Team and Hospital Transition Support Unit

The State Hospital Discharge & Diversion (SHDD) and the Hospital Transition Support Unit (HTSU) were established in response to delays in state hospital transitions. ALTSA received additional funding to address the state hospital transition and diversion needs for individuals who have been determined financially and functionally eligible for Long-Term Service Supports through increased staffing and service supports. The goal of the SHDD and HTSU teams are to support safe, person-centered transitions for individuals into their preferred setting. These goals are met through pre-transition planning and coordination, case consultations, risk reviews, staff and provider training and post-transition check-ins.

Each region has an assigned HTSU Behavior Support Trainer to offer a variety of supports for providers supporting individuals who have transitioned from State Hospitals. 

HTSU Behavior Supports

The HTSU training Behavior Support Training team provides monthly instructor-led trainings for ALTSA contracted providers and pre & post-transition consultations supports for individuals who have transitioned or diverted from Eastern or Western State Hospital.  A list trainings and consultation supports can be found in the SHDD Training, Supports, & Consultation Catalog.

Consultation Supports

To request consultation supports, contact your assigned HCS Case Manager to complete a referral form.  The HTSU Behavior Support Trainers do not provide crisis services. If you or an individual you support are in a crisis situation, contact your local crisis line or call 911.

Monthly HT Training Opportunities

To earn CEU credits for all trainings the following requirements must be met:

  1. Attendance for the full duration of the course
  2. Participation in all activities
  3. Completed an evaluation after completing the course
