DSHS moves to revoke certification of drug/alcohol treatment program facility in Pierce County

OLYMPIA-The Department of Social and Health Service notified A Fresh Beginning treatment facility in Tacoma, Washington that it has begun the revocation process on the facility’s certification.

The revocation action comes after the Department of Health found that Clarence F. Farmer, administrator and owner of A Fresh Beginning (which was formerly called A Change Counseling), had committed unprofessional conduct and suspended his credential as a chemical dependency professional for at least three years.

DSHS is required to revoke a provider’s certification when a current owner or administrator has committed unprofessional conduct.

Farmer has 28 days from receipt of the notice, sent to him via certified mail on Oct. 9, to contest this decision.

The DSHS Division of Behavioral Health and Recovery licenses and certifies treatment programs and regulates agencies that provide services for chemical dependency, community mental health, voluntary and involuntary commitment services and problem gambling.
