Visual Communications Request Form

 

Use this online form to request design, production, and revision of a DSHS publication or other communication projects from the Visual Communications (VC) office.

PLEASE NOTE:
If your project requires printing, translation, or purchasing, please use the Communication Request Form in Word or PDF formats. We require the authorizer's budget account codes and signature to process those requests. 

The information you give us will assist you and the VC team in developing a project that is high quality, meets your communication goals, and reflects the DSHS Mission. If you have any questions, please contact us at VisualCommunications@dshs.wa.gov.

You can submit this via the previous Word format and then email it to VisualCommunications@dshs.wa.gov.
