Approval for Testing Process

If a candidate trained in Washington:

After successfully graduating from a licensed training program, the candidate may apply to take the Washington state competency exam up to four times. There is no time limit on when a candidate may apply to take the competency exam. If a candidate is unsuccessful after four attempts they must retrain and retest.

If a candidate trained in another state:

Fax or mail Certificate of Completion / training certificate to the DOH NAC Credentialing, PO Box 47877, Olympia, WA 98504-7877 with their mailing address, phone number and the breakdown of clinical and classroom hours.  If they don't have this information on the certificate then they could request this information in a form of a letter and have instructor sign it.

If you are a nursing student:

Please mail an official transcript with grades for approval to the DOH NAC Credentialing, PO Box 47877, Olympia, WA 98504-7877. Nursing students may be able to sit for the exam if they have successfully completed classes that qualify them to test.

If a nursing assistant certified has not worked as a caregiver in the past 24 months and desires to work in a nursing home, they will need to retrain and retest.

If a nursing assistant certification is not renewed with DOH in 3 years, they will need to retrain and retest.
