Learning Objectives:

After completing this course, participants will understand the following:

  • The basic home and community based services and supports needs related older adults, persons with physical disabilities, persons with intellectual/developmental disabilities, traumatic brain injury, and persons with Alzheimer's disease or cognitive impairment, people in need of care transition services and supports and other populations
  • How culture and diversity may impact communication styles, health knowledge, beliefs  and  health behaviors related to health care and human services
  • Understand the range of verbal and nonverbal communication approaches across populations.  

Course Competencies:

Knowledge:  

  • Understands basic issues related to different populations, including older adults, persons with physical disabilities, persons with intellectual/developmental disabilities, traumatic brain injury, and persons with Alzheimer's disease or cognitive impairment
  • Understand care transition models.
  • General understanding of behavioral health issues and when to link a person  to other professionals for more in-depth information about specific services and supports
  • Understands how culture and diversity may impact communication styles, health knowledge, beliefs  and  health behaviors related to health care and human services

Skills and Abilities: 

  • Able to communicate in manner that is understood by others using accessible formats, language, adaptive tools and when appropriate, link to other professionals when special language, cultural or other issues  require additional attention 
  • Able to work with all people regardless of age, disability, or culture, and seeks advice from specialists, team members, partners when needed including when people have special accommodation needs Able to recognize signs of neglect or abuse and how to report this
  • Able to recognize that a person may need behavioral health services and be able to use the proper techniques and protocols for linking people to  behavioral health services
