Course #4: Person Centered Plan Development and Implementation (UofMN)

Learning Objectives:

After completing this course, participants will understand the following:

  • The philosophy and application of person centered practice with people of different income levels and with different support needs
  • How to use what is learned about a person to help them make decisions and assist them in creating a life they value.
  • How to balance what is important to a person and what is important for her or him in determining options and making decisions about services and supports.
  • How to help keep people and families engaged in the decision-making and planning process
  • How to help people develop and follow-through on short-term action plans and longer term person-centered plans

Course Competencies:

Knowledge:  

  • Understands philosophy and importance of person centeredness, self-direction/self-determination and independent living and  importance of each person directing  the planning process  
  • Understands how to help people meet immediate needs and how to approach needs that require additional discussion and planning
  • Understands how and when to involve family, caregivers, and community may provide important  informal and formal supports
  • Understands how to help individuals and families identify and work through differences and conflicts
  • Understands how to use information to develop a person-centered plan

Skills and Abilities: 

  • Able to gather, organize all information needed for a person-centered, strength based assessment.
  • Able to identify and discuss resources that most closely suit the person's goals, needs, and preferences
  • Able to help people identify risk and consider mitigation strategies
  • Able to help people identify potential problems with particular options and identify alternatives
  • Able to facilitate  agreement among all involved on course of action using tools and techniques such as motivational interviewing, problem resolution, including conflict resolution involving family members, and decision support making
  • Able to generate a written plan based on the person's self-defined goals, strengths, values and preferences.
  • Able to communicate key aspects of the plan and the person-centered description to partners in the NWD System
  • Able to communicate in a manner that is understood by others using accessible formats, language, adaptive tools, and when appropriate, involve other individuals when special language, cultural or other issues require additional attention.
  • Able to work with people to revise and make modifications to their plans over time
