Learn what other states are doing to integrate care
The Integrated Care Resource Center serves as a central location for ideas and proposals for innovative solutions for delivering coordinated health care for Medicaid's high-need, high-cost beneficiaries, with the goal of improving the quality and reducing the costs of care. The resource center is a joint project of the Centers for Medicare & Medicaid Services (CMS) Medicare-Medicaid Coordination Office and the Center for Medicaid and Children's Health Insurance Program (CHIP) Services.
