Aging & Disability Resource Centers (ADRCs) - Grant & Project Information

In 2005, Washington State was awarded an initial ADRC grant jointly funded by the Administration on Aging (AoA) and the Centers for Medicare and Medicaid Services (CMS). Through that grant, the state successfully piloted a well-functioning ADRC in Pierce County in collaboration with the local Area Agency on Aging and its partners.  

Since then, the state was awarded in 2009 an ADRC Expansion grant, expanding to 15 additional counties; and with the help of the statewide ADRC Planning and Policy Committee developed a five year expansion plan. In  2010 the state was awarded, an AoA-funded Evidence-based Care Transition Grant resulting, with the help of Qualis Health, in the Care Transition Intervention® (CTI) model being implemented by two Area Agencies on Aging and then expanded almost statewide.  Qualis Health also assisted ADSA in the creation of a state-specific CTI Implementation Tool Kit.  

Also in 2010, supplemental funding was awarded through CMS' Money Follows the Person grant to assist in the procurement of a statewide ADRC information system. In 2012, Washington State was one of 8 states awarded the Administration on Community Living (ACL; includes AoA) Enhanced ADRC Options Counseling grant that will help move the state to full statewide expansion and sustainability.  The items on this page include documents developed both with grant and non-grant funding.
